Using outdated Discovery Health Medical Scheme application forms
Discovery Health Medical Scheme will no longer accept older versions of application forms from 1 April 2018. Forms from 2017 will be accepted until 31 March 2018 to allow for older versions of forms to be phased out.
Simply refer to the year that appears in the main heading of each form to check whether you have a current form. You can find the latest 2018 Discovery Health Medical Scheme forms on the Employer Zone at www.discovery.co.za. Please make sure that you use the 2018 versions of application forms to avoid application delays.
